Main changes:
 * Final n value (2^64-1) is reserved for future use
 * Premessage patterns are "e, s" instead of "s, e" for consistency
with message patterns
 * Notion of "dependent" patterns, so MixHash(e) occurs in fallback PSK case
 * Notion of pattern "transformations", so we have a naming convention
for new patterns
 * Better acknowledgements for previous work
 * Better rationale and security considerations
