[noise] Hybrid Forward Secrecy, version 1draft-2

I have updated the extensions for hybrid forward secrecy and New Hope:

The main changes are:

- Previous review feedback was incorporated.
- The "f" and "dhff" tokens have been replaced with "f", "g", and "fg" as
discussed in the other thread.
- Full list of transformed patterns.
- New Hope is now only specified for hybrid forward secrecy, not for use
with "NN" as a basic DH function.
